Purpose: Build the nationwide targeting frame for local, in-person HRT/TRT/pellet clinics (NOT telehealth), and run a free browser spot-check of the cities where Annette has boots-on-the-ground network access for client #1.
Prior finding this builds on (ai-whitespace-EXTENDED-test-2026-06-01.md): For local HRT/TRT/pellet queries, AI surfaces brick-and-mortar clinics everywhere. Telehealth brands (Hims, Defy, Hone) never appear in local-intent answers. The real competitor is the multi-location TRT franchise (Gameday Men's Health, Ageless Men's Health) that crowds the map pack where it has several locations. So "market openness" = franchise-light / independent-heavy. Nashville and Boulder read open; Tampa (organic 1,000-review giants) and Austin (franchise lock) read tight.
1. Top US metros by population, with warm-access flags
Population figures are approximate metropolitan statistical area (MSA) populations rounded to the nearest 100K. Openness is left blank (TBD via the DataForSEO franchise-density engine once funded) EXCEPT for the six metros already sampled in prior tests, and the five warm-access cities scored below in Section 2.

Note on Greenville SC: it lands around rank ~60 by MSA size and is included explicitly per the brief. Boulder CO is well below the top 100 by MSA size (~330K) but is included as a warm-access home market and was sampled in prior tests.
2. Warm-access city spot-check (free browser, Perplexity)
Method: queried Perplexity (free, via URL-encoded search links, captured as page text) in Annette's logged-in Chrome session. Queries: "best HRT clinic [city]" and "testosterone therapy [city]". No DataForSEO spend. Each answer returned either a Google-Maps "Places" widget, a prose answer with cited sources, or both. Franchise watch: Gameday Men's Health, Ageless Men's Health, Renew Vitality (a national TRT chain that recurs).

Read
- Telehealth fear stays unfounded across all five: every answer was map-pack/local-clinic driven. Zero Hims, zero Defy, zero Hone.
- Renew Vitality is present in all five cities but as a single location in a long list, never dominating. It is a national TRT chain but it does not crowd the way Gameday does when Gameday plants multiple sites.
- Gameday Men's Health is the tell. It is absent from DC, Boston, St. Louis; appears once in Miami; appears TWICE in Greenville. The two-location Greenville footprint is exactly the franchise-crowding pattern that made Austin and Phoenix tight in the prior test.
- St. Louis is the most clearly independent-heavy of the five: a deep bench of local names and a 20-year local pellet incumbent (BioBalance) as the prose anchor, with only one franchise location anywhere in the answer. There is also a clear menopause/women's BHRT lane separate from the men's TRT lane, which doubles the openings.
- No city showed a single cost-authority source (unlike the Boulder dentistry/orthodontics findings). HRT openings here are discovery/best-of and review-velocity plays, not cost-page plays.
3. First-target shortlist (warm-access AND open)
Cities that are both warm-access AND read open from the spot-check, ranked by how clean the opening is for client #1. The ranking weights franchise-lightness, depth of the independent bench, and the presence of distinct men's-TRT vs women's-BHRT lanes (more lanes = more sellable openings).
- St. Louis MO (daughter) , Best opening. Deepest independent bench, only one franchise location in the entire answer, a long-standing local incumbent (BioBalance) that anchors prose without locking it, and two distinct lanes (men's TRT and women's/menopause BHRT). Boots on the ground via daughter.
- Washington DC (brother) , Strong. No Gameday or Ageless at all, only Renew Vitality. Top name (Ritual, 407 reviews) is an independent med spa, beatable on a focused HRT/GBP play. Large, high-value metro (#7 by population).
- Boston MA (sister) , Strong and clean. Franchise-light (only Renew Vitality), independents lead the Places widget, none with a dominating review moat. High-value #11 metro.
- Miami FL (daughter) , Good but slightly more contested. Independent-heavy with a deep bench, but Gameday has a foothold and several incumbents carry mid-hundreds review counts. Open-to-mixed; workable, just not as clean as the top three.
Held back from the shortlist: Greenville SC (daughter) , MIXED, not open. Gameday's two-location footprint crowds the men's/TRT map pack the way it locks Austin. The women's BHRT lane (The Danita Clinic, Biltmore, Forum Health) is still independent and winnable, so Greenville is a viable women's-HRT-only target, but it is not a clean general-HRT opening.
Recommendation: Pursue client #1 in St. Louis (cleanest opening + family on the ground), with Washington DC and Boston as the immediate backups. All three are franchise-light, independent-led, and have a warm intro path.
4. Pending: full 100-metro openness scoring
Openness for the ~90 metros still marked TBD is pending the DataForSEO franchise-density heat-map engine (a scan that scores each metro by how many Gameday / Ageless / other multi-location TRT franchise locations occupy the local map pack versus independents). That engine is NOT run here: the DataForSEO balance is low (~$0.83) and is being reserved. Once the account is funded, the engine should:
- Score every top-100 metro on franchise density (franchise map-pack share vs independent share).
- Flag separate men's-TRT and women's-BHRT openness where the lanes diverge (as they did in St. Louis and Greenville).
- Cross-check against the warm-access spot-check above to validate the cheap browser read against the paid map-pack data.
- Output a single ranked openness column to fill the TBD cells in Section 1.
Until then, only the six prior-test metros and the five warm-access cities carry a real openness verdict. Everything else is a population placeholder awaiting the funded scan.
