Patient: Annette Thompson. Prior draw: 2026-05-15 (Quest plus Cleveland HeartLab Cardio IQ). Requested: one early-morning fasted draw.

Quest codes are given so the requisition can be built directly. She pays out of pocket, so please order through whichever route prices lowest on your side.

What each one answers

Change from May: the testosterone code

May used Quest 14966, where free testosterone is calculated from total, SHBG and albumin. This order uses 37073, where free testosterone is measured by equilibrium dialysis, total stays on mass spectrometry, and SHBG is included.

Two consequences:

  1. The dialysis value is the more rigorous measurement.
  2. Because the methods differ, the first 37073 result resets the free-testosterone baseline rather than continuing the May trend line. Total testosterone and SHBG stay directly comparable.

37073 does not include bioavailable testosterone, which was 102.9 ng/dL in May. If you want that retained, 14966 is the code.

Dropped, in case you disagree

TMAO. Adjusting for renal function attenuates or reverses the TMAO to cardiovascular association, and her eGFR is 77, so an elevated result would be hard to attribute. She is also deliberately raising oily fish intake to lift OmegaCheck, which raises TMAO directly. No AHA or ACC recommendation for routine screening, and no established treatment target.

NT-proBNP and vitamin B6 (PLP), both on cost.

Preparation and reading notes

  • Fasted, early morning. One draw covers all nine.
  • Read hemoglobin and hematocrit against altitude-adjusted ranges. She lives at 7,700 feet, and sea-level ranges overstate the finding.
  • Given HFE C282Y status and active ferritin recovery, therapeutic phlebotomy is not an appropriate response to the hematocrit.
  • She has no health insurance. Every test is out of pocket, so cost is a real constraint on what goes on the requisition.
