Evolution Dermatology: Candid Opportunity Assessment
The question: Can SEO Believer genuinely improve this practice's visibility, or are they already strong enough that pitching risks overpromising?
Method: Re-read all prospect files (June snapshot, 50-call AI citation run, prospect record, handoff draft), then fresh live checks today: fetched the homepage HTML and parsed it, checked robots/sitemap/llms.txt, and ran web searches for reviews and third-party "best dermatologist Boulder" coverage.
1.Scorecard: where Evolution already wins (what Annette can't improve)
These are genuinely strong, and the pitch should say so out loud:
- Reputation is elite. Roughly 160 to 163 Google reviews at a 5.0-star rating (confirmed today across MedSpaScout, Portrait, Medical Spa Locator, Trustindex). Most practices never hit 5.0 at that volume. There's nothing to fix here.
- Map Pack presence. Per the June geo-grid read, Evolution already shows in top local results across most of Boulder for dermatology and skin-care searches. That's the hardest local asset to build and they already have it.
- Site fundamentals. HTTPS, fast (homepage returned in about 0.3 to 0.6s today, 419KB), mobile viewport present, all homepage images have alt text, robots.txt and sitemap.xml both return 200, reviews surfaced on-page, clean NAP on the homepage.
- Real demand engine. A liked NP, a Weave booking system, loyal long-tenure patients (testimonials cite 10 to 20 year relationships). The business itself doesn't need saving.
Bottom line on strengths: this is not a broken business and not a broken website. Anyone pitching "your site is a mess" would be lying.
2.Measurable gaps (verified still live today, 2026-07-08)
Every technical issue flagged in the June 9 snapshot is still on the live site a month later. Nobody's fixed anything, which itself tells you they have no one working on this.
Gap A: The AI citation gap (the headline)
- Evidence: Clean 50-call run (2026-06-10, zero refusals): Evolution CSI 2.5/100, 4th of 4, named by only Perplexity. Boulder Valley Center for Dermatology (19.8) and Dermatology Specialists were named by 5 of 5 AIs. Raw data lives in
FindWebSiteClients/outputs/evolution_cite_*-2026-06-10.json. - Why it exists (confirmed today): Evolution is absent from the third-party sources AIs retrieve from. Yelp's "Best 10 Dermatologists in Boulder" list (updated April 2026) names Azeal, Kallgren, Rocky Mountain, Mandel, Boulder Valley, and Ideal, but not Evolution. No independent "best of Boulder" listicle names them. AIs recommend what the corroborating web says, and the corroborating web is silent on Evolution.
- What fixing it does: Cash-pay cosmetic patients increasingly ask ChatGPT and Perplexity directly. Going from 1-of-5 to 3-of-5 AIs puts Evolution in front of the exact high-LTV patient (Botox, filler, laser) who skips insurance directories.
- Confidence it's movable: Medium The inputs (schema, entity consistency, third-party citations, review-site presence, structured content) are all buildable in a Sprint plus 2 to 3 months of monitoring. But AI retrieval refresh cycles aren't under anyone's control; promising a specific CSI number by a specific date would be overpromising. Promise the inputs and the measurement, not the output date.
Gap B: No local/medical structured data
- Evidence (fetched today): JSON-LD on the homepage contains only generic Organization, WebSite, WebPage, BreadcrumbList. No MedicalBusiness/MedicalClinic, no address, phone, geo, hours, or services in machine-readable form. The fix is already pre-built (
evolution-dermatology-schema.txt). - Impact: This is the single clearest machine-readable signal a local practice can hand Google and the AIs. Directly feeds Gap A.
- Confidence: High Fully deliverable in week one of a Sprint. Zero dependency on third parties.
Gap C: On-page basics still broken
- Evidence (fetched today): Duplicate H1 "Better Skin Health is Our Mission" appears twice, no city in it. Title is still the 32-character "Evolution Dermatology Boulder CO". Footer still says "Copyright 2022". Zero embedded Google maps. No llms.txt (404).
- New finding today: the meta description still advertises "IV therapy", a service Annette's own June verification confirmed they do NOT offer. Their front door is telling Google and every AI a wrong fact about their own services. That's a concrete, undeniable, 5-minute demo of "the machines have bad data about you."
- Confidence: High All fixable in days. Individually small, collectively they're the entity-consistency layer the AIs read.
Gap D: Third-party citation and review-site footprint
- Evidence: Thin Yelp presence (their Yelp page exists but they're not in the top-10 list), a Fresha aggregator page with wrong data floating around unclaimed, no presence in local editorial roundups.
- Impact: This is what actually moves AI citations over 90+ days. It's also the part that takes sustained monthly work, which is exactly what the $1,500/mo Monitoring is for.
- Confidence: Medium-High for building the footprint; Medium for how fast the AIs re-crawl and reflect it.
3.Revenue logic
A cosmetic dermatology patient in a market like Boulder is not a $150 visit:
- Botox: roughly $400 to $700 per session, 3 to 4 sessions a year, so $1,200 to $2,800/yr.
- Filler: $600 to $1,000+ per syringe, often 1 to 2 a year.
- Laser/IPL (they run Lumenis M22, CO2, Opus, Sofwave): packages commonly $1,500 to $3,000.
A retained cosmetic patient is conservatively worth $1,500 to $3,000 per year, and these patients stay for years (their own testimonials cite 10 to 20 year relationships), so lifetime value plausibly runs $5,000 to $15,000.
The math against the fee: one net-new cosmetic patient per month is roughly $18,000 to $36,000 in first-year revenue. Year-one cost of the engagement (a $4,500 Sprint plus $1,500/mo) is about $22,500. So the whole thing pays for itself at roughly one extra cosmetic patient a month, and everything past that is margin. That's a defensible bar, not a fantasy one.
4.Bottom-line verdict: HONEST PITCH
Annette's instinct that "they're already good" is half right, and that half is exactly why the pitch works. They've won the parts that are hard (reputation, map pack, patient loyalty) and lost the part that's new (AI recommendations), and the June data proves it with numbers, not vibes. The gaps are real, still live a month later, and mostly within her direct control. What's NOT fully in her control is how fast five AI systems update, so the framing must be "we fix the inputs and measure monthly," never "you'll be named by ChatGPT by September."
This is not a DON'T PITCH. A practice with a 2.5/100 citation share while two competitors sit at 5-of-5 AIs is underserved by definition. And it's not quite a STRONG PITCH only because the flagship outcome (AI citations) has a lag she can't schedule.
The 3 claims Annette can make without overpromising
"When we asked all five major AIs the questions Boulder patients actually ask, you were named by one of the five. Two of your competitors were named by all five. I have the 44 recorded answers." Measured fact, reproducible, hers to show on a Loom.
"Your website currently tells Google and the AIs that you offer IV therapy, which you don't, and it has none of the machine-readable data saying you're a dermatology clinic in Boulder. I've already built the corrected data file; it deploys in week one." Verified live today; the fix exists and is fully in her control.
"For a practice where one cosmetic patient is worth $1,500 to $3,000 a year, this engagement pays for itself at about one extra patient a month. I'll send you the citation measurement every month so you can see whether it's working, and you can stop any time it isn't." Ties fee to a conservative bar and makes the monitoring the accountability mechanism.
One honest caveat to keep in the back pocket
If the practice is already at capacity (a liked NP with a full book), more demand is worth less to them. The pitch should target the cosmetic/wellness side, which cash-pay practices essentially always want more of, and the discovery call should ask about capacity before promising growth.
Addendum: Med Spa & Ownership Findings, 2026-07-08
Method: Live fetch of evolutiondermatology.com (home, services, about, contact, cosmetic pages), LinkedIn company data via search, and fresh Google searches on four Boulder med spa money queries plus multi-state location searches.
A1.Hypothesis 1: Part of a larger group / multi-location? REFUTED
Evolution Dermatology + Wellness is a single-location, founder-owned micro practice. Evidence:
- One address everywhere: 1455 Yarmouth Ave, Suite 112, Boulder, CO 80304. The site's contact page lists only Boulder; there's no locations page, and every page title says "Boulder, CO."
- LinkedIn company page: 2 to 10 employees, privately held, founded 2022, Boulder CO. No parent company listed. Their one visible job posting (a Medical Aesthetician role) is posted by the practice itself, not an MSO.
- Founder-owned: the about page and homepage present Pauli Morrow, MMSc, PA-C as the founder of Evolution Dermatology, with a small team (Kim Guthke PA-C, an aesthetician, front office). No physician group branding, no MSO footer language, no "part of" anywhere.
- Multi-state searches ("Evolution Dermatology" excluding Boulder, plus Texas/Arizona/Florida variants) surface nothing but the Boulder practice. The only lookalike is Evolve Dermatology in Texas, an unrelated business with a different name.
Location list found: Boulder, CO. That's it.
A2.Hypothesis 2: Med spa service line? CONFIRMED, and it's baked into the brand
They aren't a medical derm shop with a med spa bolted on; the legal-facing name is "Evolution Dermatology + Wellness" and Yelp categorizes them under Medical Spas, not dermatologists. Confirmed cosmetic services (from the live services page and cosmetic dermatology page):
- Botox / Xeomin neurotoxins (dedicated page)
- Juvederm / Restylane injectable fillers (dedicated page)
- IPL and laser/light procedures (Lumenis M22, CO2, Opus, Sofwave per the June audit)
- Chemical peels, cosmetic consultations, advanced skincare
- Evolution VIP Membership: monthly treatments, skincare savings, "special Botox bonuses" (recurring cash-pay revenue, promoted on the homepage)
- They're on Allē (Allergan's Botox loyalty network), which only active injectable practices join.
No separate med spa brand or sub-site; it all lives on the one domain. No CoolSculpting found.
A3.Hypothesis 3: Med spa visibility gap? CONFIRMED, and it's stark
Fresh searches today on the four money queries. Evolution appears in zero of the organic winner sets:

Why: the winners all run treatment-plus-city landing pages ("Lip Filler Boulder, CO", "Best Botox Boulder"). Evolution has generic service pages ("Neurotoxins", "Injectable Fillers") with no Boulder money-query targeting, no schema, and no presence in the Yelp/RealSelf/editorial layers those results pull from. This is the same root cause as the AI citation gap in the main assessment (CSI 2.5/100), now shown to also cost them the cash-pay searches.
The delicious detail: on MedSpa Scout's Boulder ranking they're literally the highest-rated med spa in Boulder. Their reputation already won the category; their visibility just never showed up to collect.
A4.Hypothesis 4: Decision-maker? Simple, and in Annette's favor
There's no corporate layer. Pauli Morrow is the founder AND the injector Annette knows. Marketing spend for a 2-to-10-person founder-owned practice is decided by the owner, possibly with the office manager (June notes reference "Kim" as the Loom addressee; confirm whether that's Kim Guthke PA-C or a front-office Kim). No procurement, no regional marketing director, no "I'll have to ask corporate." One warm conversation with Pauli is the whole pitch path.
A5.Revised revenue logic
The main assessment priced a cosmetic patient at $1,500 to $3,000/yr. The med spa findings strengthen it two ways:
- The VIP Membership converts a won patient into recurring revenue (monthly treatments plus Botox bonuses), so a captured med spa patient is a subscription, not episodic visits. Membership-model med spa patients plausibly run $2,400 to $4,800/yr.
- The gap is on the highest-margin queries. "Botox boulder" and "lip filler boulder" searchers are cash-pay, pre-qualified buyers. Every one of those searches today lands on Mov, Luxe Room, Vasu, or Youthbar. The break-even bar from the main assessment (one net-new cosmetic patient per month covers the whole year-one fee) holds, and the med spa angle makes it easier to hit because the target queries are commercial-intent, city-level, and currently uncontested by Evolution's own pages.
A6.How this changes the pitch
Verdict: HONEST PITCH holds, and gets sharper. Not upgraded to STRONG PITCH (the AI-citation lag caveat from the main assessment still applies), but the framing improves:
- Kill the "corporate" worry. No MSO, no other locations, no exclusivity conflict beyond Boulder itself. Market-exclusivity rule: signing Evolution locks the Boulder dermatology AND Boulder med spa slots, so factor that in (they'd be a good practice to spend both slots on).
- Lead with the med spa gap, not the derm gap. "You're the highest-rated med spa in Boulder on the rating aggregators, and you don't appear anywhere when someone Googles 'botox boulder' or 'lip filler boulder.' Mov, The Luxe Room, and Vasu are taking those patients." That's more visceral than the AI citation share, and it's the cash-pay, membership-converting side of their business.
- Concrete deliverables write themselves: Boulder-specific treatment landing pages (Botox Boulder, Lip Filler Boulder, Laser Boulder), MedicalBusiness + MedSpa schema, Yelp/RealSelf/editorial citation building, VIP Membership as the conversion offer on those pages. All within a Sprint's direct control.
- Pitch path: straight through Pauli. She's founder, owner, and decision-maker; there's no one behind the curtain. The discovery question from the main assessment (capacity) matters even more now: ask specifically whether the cosmetic/injectable book has room, because that's the side this work fills.
