What's Actually Going On

Update: You Ran the Test, and You Have Your Answer

The doubling disappears when you cover either eye. That is binocular diplopia. Your eyes are pointing at slightly different places, and that is a fixable alignment problem.

Here is what that rules in and rules out. Your two eyes each see one clean image, so nothing is splitting the picture inside either eye. The problem is that the two images arrive at your brain out of register and it cannot fuse them. That puts you squarely in the world of prism, botulinum toxin, and strabismus surgery, which is the world with the published success rates in Section 3, running from 62% to 93%.

It also means your early cataract and your two LASIKs are not what is causing the doubling you are describing. They still matter and still need documenting (Section 6), but they are a separate track. Had the doubling persisted with one eye covered, prisms and vision therapy would have been useless to you and this would have become a cornea-and-lens problem instead.

Binocular, worse at distance, female, 57, needing prism for driving: that is the textbook profile of sagging eye syndrome, the single most common cause of new double vision in adults over 40. Section 2 is now the section that applies to you.

One honest caveat so you do not over-read this. The home cover test is a screening observation, not a measurement. It does not tell you which eye is drifting, how big the deviation is in prism diopters, whether there is a vertical component (65% of sagging eye syndrome cases have one), or which specific binocular cause this is. It also cannot rule out a small monocular component sitting underneath, which your cataract and LASIK history make plausible. What it does do is confirm your appointment is aimed at the right target.
And you booked it the same day. Colorado Center for Eye Alignment, Erie office. That is exactly the right clinic for a confirmed binocular deviation, and it is about 20 minutes from you. Skip to Section 8 for what to bring and what to write down beforehand.

Start Here: The Headline

Your working theory is that screens weakened your eye muscles. The evidence points somewhere else, and honestly, somewhere better. You needed prism for driving, which means your double vision is worse at distance. Screens are a near task. If near work were driving this, you'd see double at the computer and be fine in the car. You describe the opposite.

That reversal matters, because it points at a specific, common, well-studied condition in women your age, and it has treatments with published success rates of 62% to 93%. It also means the online eye exercises you were considering are aimed at a different problem than the one you most likely have.

There was a free test to run first, before spending a dollar, that decided which half of this document applies to you. You ran it, and the answer is above.

The Test You Ran, and Why It Mattered

Look at something doubled, roughly 20 feet away. Cover one eye. Then the other. This single observation splits every possible cause into two completely different treatment worlds. You ran it: the doubling disappeared. You are in the first world below.
  • ✓ YOURS: the doubling disappears when either eye is covered. This is binocular diplopia. Your eyes are pointing at slightly different places. Prisms, botulinum toxin, and eye muscle surgery all work on this. This is the world your prism glasses came from.
  • Not yours: the doubling is still there with one eye covered. This is monocular diplopia, an optical problem inside that one eye. Prisms do nothing for it. Vision therapy does nothing for it. It gets fixed at the level of the cornea or the lens.
Keep this in view anyway. You have two independent risk factors for the monocular kind: your early cataract, and two rounds of LASIK. Both are documented causes of ghosting in one eye. Your test says they are not driving the doubling you notice now, but a small monocular component can sit quietly underneath a binocular one, so ask the specialist to check for both rather than stopping at the first answer.

Three more things to note before the appointment, because the specialist will ask and they narrow the diagnosis: are the two images side by side, or one above the other? Is it worse late in the day than first thing in the morning? Does tilting your head help? Full prep list in Section 8.

The Most Likely Diagnosis: Sagging Eye Syndrome

In 2019, the Stein Eye Institute at UCLA published a review of 945 consecutive adults over 40 who showed up with new binocular double vision. They asked a simple question: what actually causes this?

The single most common cause was sagging eye syndrome, at 31.4%. That's three times more common than the next cause on the list. It is more common in women. And its share of all double vision climbs steeply with age.

What is actually sagging

Here's the part that reframes your whole question. Your eye muscles run through connective tissue sleeves called pulleys, which hold each muscle in the right position the way a guide ring holds a fishing line. One of these, the band ligament connecting the lateral rectus to the superior rectus, thins out with age. MRI studies confirm it. When it thins, the muscle's pulley slips downward.

The muscle is not weak. The suspension has stretched. That is a completely different mechanical problem, and it is why "strengthening exercises" are the wrong tool. You cannot do repetitions to shorten a stretched ligament back into position, any more than you can do calf raises to fix a fallen arch.

When both sides sag evenly, the eyes drift slightly outward-facing-inward at distance, producing exactly the pattern that needs prism for driving and no prism for reading. When one side sags more than the other, you get a small vertical misalignment. Two thirds of the sagging eye syndrome patients in the study had a vertical component.

The menopause connection nobody told you about

The study authors raise something that should interest you specifically. The orbital pulley system is built from collagen, elastin, and smooth muscle. Estrogen and progesterone help preserve collagen by slowing its degradation. Those hormones fall after menopause. The authors offer that as the most plausible explanation for why sagging eye syndrome hits women harder: 59.9% female in their sagging eye group versus 52.0% in everyone else, and other series report 69% to 85% female predominance.

To be clear about what this is and isn't: that is a proposed mechanism, not proven causation, and no study has tested whether HRT changes sagging eye syndrome risk. But given you're already on HRT and already think hard about collagen and menopause, it's a genuinely interesting question to put to a specialist rather than one to answer on your own.

What Actually Works, With Real Numbers

The Pediatric Eye Disease Investigator Group ran a prospective study across 28 clinical sites, following 110 adults with exactly this distance-worse pattern. Median age 71, 67% female. They tracked what treatments doctors chose and whether the double vision actually went away.

Success meant reporting double vision "rarely" or "never" when looking straight ahead into the distance. In other words, the thing you want back.

The number that answers your exercise question
Of those 110 adults, 68% were treated with surgery and 29% with prism. Three people, 3%, were prescribed divergence exercises. That was too few for the researchers to even analyze. Across 28 expert sites, essentially nobody treats this pattern with exercises, because there is no evidence base saying it works.
What surgery actually looks like now

This is not the operation you might be picturing. In the Stein Eye series, 149 patients had surgery. Average distance misalignment went from 6.9 prism diopters down to 0.3. Surgery resolved distance double vision in every single case. Reoperation rate was 13.4%, and there is roughly a 20% recurrence rate at around two years, so it's not necessarily forever.

  • Adjustable sutures are standard. The surgeon can fine-tune the alignment afterward while you're awake and can report what you see.
  • Topical anesthesia is often enough. Surgeons frequently pick medial rectus recession over lateral rectus resection specifically because it's more comfortable awake.
  • For a small vertical component, graded vertical rectus partial tenotomy corrects up to 10 prism diopters and is less invasive than full-tendon surgery. It was used in 72% of the vertical cases.
  • Botulinum toxin is a third option, used both as treatment and as a preview to test whether surgery would give you a result you'd be happy with.
The cheap first step
In the PEDIG study, 53% of prism patients started with press-on Fresnel prisms: a thin plastic sheet that sticks onto your existing lens. Cheap, reversible, and it tells you within days whether prism restores comfortable distance vision at your current misalignment. Ask for this before anyone grinds anything or schedules anything.

Where Vision Therapy Genuinely Works

Vision therapy has real, high-quality randomized evidence behind it. It's just for a different condition: convergence insufficiency, where the eyes drift apart at near. That's the opposite direction of drift from the age-related distance pattern.

The CINAPS trial randomized 50 adults to office-based vergence therapy or a convincing placebo therapy, 12 one-hour sessions, with masked examiners. Results:

  • Near point of convergence improved 6.0 cm versus 3.1 cm for placebo (p < 0.01).
  • Convergence range improved 17.3 versus 7.4 prism diopters (p < 0.001).
  • 72% versus 32% normalized both objective measures.
  • But symptoms improved about equally in both groups (12.4 versus 10.1 points, p = 0.56). The authors concluded the symptom questionnaire itself needs revising.
  • Home therapy adherence was 24%. The authors' own conclusion: patients are unlikely to comply with home-based therapy, and that's likely why home programs underperform supervised ones.
Read that honestly. Office-based therapy really does move the objective binocular measures in the right patient. The subjective symptom benefit is murkier. And the home-program compliance data is a warning about the exact category of product you were considering buying.
The home and telehealth landscape, straight
  • Vivid Vision Home is the most legitimate of the VR options. Prescription-only through a provider, indicated for convergence insufficiency, amblyopia, and strabismus. A pilot randomized trial found VR therapy comparable to office-based therapy for convergence insufficiency. Pilot-scale, young-adult population.
  • Bynocs and AmblyoPlay are consumer app programs marketed for lazy eye and convergence problems. Weaker independent evidence.
  • Remote therapist-supervised programs exist, including at Denver's Neuro-Vision Therapy Institute. The AAPOS adult strabismus telemedicine review found telehealth useful for screening and follow-up of established patients, mainly to shorten the wait for a subspecialist. It is not a substitute for the first exam, because measuring your misalignment is a hands-on prism test.
None of these will hurt you. But every one of them targets near-vergence mechanics. Buying a headset before you've been measured is spending money on the wrong target. Get measured first. If the measurement shows a near component, the therapy option is right there in Boulder.

What Else It Could Be, and What Needs Ruling Out

Sagging eye syndrome is the leading candidate, not a conclusion. Here's the rest of the field from that same 945-patient series, plus the ones that matter most for you.

Other alignment causes worth naming
  • Decompensated phoria, possibly LASIK-related. There's a documented literature on binocular decompensation and double vision after refractive laser surgery, and on monovision correction degrading fusion. Find out whether either of your two LASIK procedures was set for monovision. That's a specific, answerable question and it could be central.
  • Fourth nerve (trochlear) palsy, 10.3% of cases, including a mild congenital one that quietly decompensates in mid-life. Note the classic three-step test used to diagnose it is only about 70% sensitive and 50% specific, and sagging eye syndrome is frequently mislabeled as this.
  • Convergence insufficiency exotropia, 5.1%. This is the one vision therapy is built for.

Red Flags: These Need a Doctor, Not Exercises

  • Thyroid eye disease (8.3% of adult double vision). Watch for bulging, lid retraction, puffiness around the eyes. You already have thyroid testing on your open list. Connect these two.
  • Myasthenia gravis (2.1%). Watch for double vision that varies, gets worse late in the day, comes with a drooping lid, or has spontaneously resolved before. It can imitate almost any pattern.
  • Microvascular cranial nerve palsy, the most common cause of eye movement paralysis over 50. Usually clears in about three months on its own.
  • Sudden onset, pain, a changed pupil, or any other neurological symptom. That's urgent imaging, same week, not a routine appointment.
Monocular causes (the ones that survive covering one eye)
  • Early cataract. A clouding lens scatters light and can split an image in one eye.
  • Post-LASIK higher-order aberrations, especially coma, which is a documented cause of monocular double vision. Two procedures means two rounds of corneal reshaping.
  • Dry eye and irregular tear film. Boring, extremely common, sometimes dramatic, and often the cheapest thing on the list to fix. Very common after LASIK.

Your Cataract Plus Two LASIKs: Plan This Now

Your cataract is early, so surgery is likely years away. But two things are worth doing while you're thinking about eyes anyway, because one of them gets harder with time.

Calculating the right lens implant power for cataract surgery is significantly harder after LASIK. Standard formulas systematically miss. The accepted tools are the Barrett True-K formula and the ASCRS post-refractive calculator, which get mean absolute error down to roughly 0.36 to 0.58 diopters in post-LASIK eyes, close to but still short of never-operated eyes.
  • Request your LASIK operative records from both procedures now. Pre-operative refraction and topography meaningfully improve the calculation, and clinics purge old records. This is a phone call that pays off a decade from now.
  • Find out if either was monovision. Relevant twice over: as a possible contributor to the double vision today, and as a decision point at cataract surgery later.
  • Do not wait for cataract surgery to deal with the double vision. Your cataract is early. Your alignment is treatable now. Those are separate timelines.
  • Sequencing note for later: if the cataract does become visually significant, it's usually addressed before eye muscle surgery, because it changes your refraction and therefore the measurements.

On driving

Colorado requires 20/40 distance acuity in at least one eye with correction and at least 120 degrees of combined horizontal field. Constant double vision is disqualifying. Double vision corrected by prism in your glasses is the standard, accepted workaround, carried as a corrective lenses restriction. Confirm the current rule with whichever clinic writes the prescription.

The point worth holding onto: you are not restricted because of an unfixable fact about your eyes. You're restricted because a treatable condition drifted past what your old prism prescription could handle. Sixty-two percent of people get their distance vision back with prism alone. Seventy-nine percent with surgery. Those are good odds on getting the car keys back.

Getting Ready for Erie

✓ Cover test, done.   ✓ Adult strabismus subspecialist, booked.   The first two steps are behind you, and they were the two that mattered most.

Bring these to the appointment

  • Your old prism driving glasses, even though you stopped wearing them, plus the prescription and date if you can find it. How much prism you needed and when is real data on how fast this has changed. If you cannot find the paperwork, call the optometrist who wrote it and ask them to pull the record.
  • LASIK operative records from both procedures. Request these now, do not wait for the appointment. Ask each clinic for the date, your pre-operative refraction, corneal topography, and the treatment parameters. These matter twice over: for the monovision question now, and for your cataract lens calculation years from now (Section 6). Clinics do purge old records, which is the only genuinely time-sensitive item on this page.
  • Your current glasses, whatever you actually wear day to day.
  • Medication and supplement list, including your HRT doses. The collagen hypothesis in Section 2 makes your hormone status directly relevant here, and most patients never think to mention it to an eye doctor.
  • Recent labs, especially your TSH. Yours ran low at 0.27 with normal T4 and T3. Thyroid eye disease accounts for 8.3% of adult double vision and should be explicitly excluded rather than assumed away.

Write these down before you go

Not from memory in the exam room. Take a week and actually watch for them.

  • Are the two images side by side, one above the other, or tilted? Vertical or tilted separation points toward a vertical component or a fourth nerve palsy, and changes what gets measured.
  • Is it worse late in the day, or when you are tired? Say this out loud at the visit if the answer is yes. Fatiguable double vision is the myasthenia gravis question, and it deserves an explicit answer before anyone discusses prism power.
  • How far away does something have to be before it doubles? Across a room, or only well beyond that?
  • Does turning or tilting your head make it better or worse?
  • Any doubling at all up close, at the computer or reading? Even slight. This is what would flag a near component.
  • Any eye pain, headache, drooping lid, or change in how your eyes look?

What happens next

  1. At the visit, get the measurement. Prism and alternate cover test at distance and near, in prism diopters, plus a plain statement of whether this is sagging eye syndrome, a decompensated phoria, a nerve palsy, or something else, and whether any monocular component is layered on top.
  2. Ask for a press-on Fresnel prism trial before anything permanent. Cheap, reversible, sticks onto your existing lens, and tells you quickly whether prism restores comfortable distance vision. Then you decide ground-in prism versus surgery from actual experience rather than a prediction.
  3. If prism is not enough, the surgical numbers are in Section 3, and the questions to ask about the surgeon's own results are in Section 9.
  4. Only if a near component turns up, add office-based vergence therapy locally, and treat any home or VR program as an adjunct to supervised sessions, never a replacement.

Questions to Take With You

  1. Is my double vision binocular, monocular, or both?
  2. What is my measured deviation at distance and at near, in prism diopters, today? How does that compare to when my old prism was prescribed?
  3. Is this sagging eye syndrome, a decompensated phoria, a nerve palsy, or something else?
  4. Have thyroid eye disease and myasthenia gravis been excluded, and how?
  5. Was either of my LASIK procedures set for monovision, and could that be contributing?
  6. Can we trial a press-on Fresnel prism before committing to ground-in lenses?
  7. If prism isn't enough, am I a candidate for adjustable-suture surgery under topical anesthesia? What is your own success and reoperation rate?
  8. Does my early cataract change the timing of any of this?
  9. What should I document now so my lens implant calculation is accurate later, after two LASIKs?
  10. What specifically would let me drive comfortably again, and how soon?
