ICAHN Scan: B2B Physician-Marketing Keywords
Run 2026-07-10 via YouTube Data API (12 keyword searches, 271 unique videos scanned, plus uploads-paging on 7 competitor channels, ~2,100 total videos checked). Method: Shane's ICAHN checklist from 03-content-genesis-system. Checklist applied: channel under 100k subs, views-to-subs ratio 5:1+ (relaxed for low-view B2B per Shane's own rule), engagement above ~1% to rule out bought views, length noted. Engagement = (likes + comments) / views, the best proxy the API gives.
What the winning ideas have in common (5-line summary)
- "Behind the curtain of running a practice" beats "marketing tips." The biggest ratios come from day-in-the-life, income-truth, and "lessons since starting my practice" videos, not from "5 marketing strategies" listicles. Doctors watch doctors talking honestly about the business, then absorb the marketing lesson inside the story.
- Concrete money and patient numbers in the title win. "Make millions at your medical clinic," "double patient enquiries in 12 months," "six figures as a part-time therapist." Vague titles ("effective strategies") die.
- The niche is starving. Almost every qualifying muse has weak packaging: stock-photo thumbnails, corporate branding, monotone delivery, some are 5+ years old and still ranking. Nobody under 100k subs is making genuinely good videos on these keywords. That's the second-mover opening.
- Practice-launch and practice-growth "how to start / how to grow" evergreen searches keep paying for years (2019-2021 videos still top the results), so Annette's videos can compound instead of decay.
- Telehealth-specific supply is nearly zero. One counseling-clinic Short (87:1 ratio) and one nurse-practitioner LLC video are the only telehealth-practice results that qualified. "Grow a telehealth practice" is wide open, and menopause-telehealth is untouched.

Honest gaps and caveats
- All 12 keyword searches completed; quota did NOT run out (about 1,400 units used total).
- No vidIQ data (no browser session in this run): search volumes and "Nx channel average" flags come from API stats, not vidIQ.
- I didn't watch first-30-seconds quality (Shane's step 7); judgments are from metadata, thumbnails weren't visually graded. Spot-check the top 5 muses by eye before scripting.
- Several top-ratio muses are Shorts; the ratio is still valid idea-proof but view economics differ. Flagged in the table.
- "Patient acquisition for doctors" and "content marketing for medical practices" returned almost no qualifying small-channel results: low supply, which is good news, but also means fewer muses on those exact phrases.

Second pass, same day, same ICAHN rules, aimed at the "found, believed, and booked online" positioning: Google Business Profile / local SEO, trust websites, Instagram for clinicians, telehealth business, and appointment-filling. 15 new keywords, each searched twice (viewCount + relevance order), 592 unique videos scanned, all deduped against round 1. ~3,000 API units used (fresh quota day, no exhaustion).

Shorts (R2-2, R2-3, R2-5, R2-7, R2-8, R2-10, R2-11) are idea-proof only, remake as 10-15 minute long-form. Excluded as suspect: two iScript GBP-for-healthcare videos (513k and 61k views on a 10-subscriber channel with 0.0% engagement, the signature of paid ad views, not organic demand).
The GBP / local-SEO gap: confirmed, with a nuance
The suspicion was right, but the shape of the gap is more specific than "empty":
- Generic GBP content has enormous proven demand: general-audience GBP videos in these same searches carry 100k-700k views (verification tips, appeal tool, profile setup), mostly from marketers with weak packaging.
- Doctor-specific GBP/local-SEO supply is close to zero: one 13-minute setup video from a 109-sub channel (R2-12), one Indian app demo, one 5-year-old MyPractice101 video stuck at 1,709 views, and one 3-week-old Short (R2-10). That is the entire field.
- "doctor website that converts" returned zero qualifying videos across 50 results. "Local seo for doctors" and "social media for physicians" barely better. The searches doctors would actually type have no one answering them.
- Honest caveat: near-zero supply also means no single mega-proof video for "GBP for doctors." The demand evidence is triangulated (huge generic-GBP demand, doctors demonstrably searching patient-growth terms, and every scrap of doctor-specific supply over-performing its channel), not a single 500k-view muse.
Combined summary after both rounds (32 muses)
- Round 1's core finding holds: behind-the-curtain practice-business stories beat marketing-tips listicles, and money-in-the-title wins (R2-11, R2-13 repeat the pattern).
- Telehealth-business content is the fastest-confirming lane. Both rounds independently surfaced telehealth muses at the top (87:1 in round 1; 242:1, 113:1, 19:1 in round 2). "Start/grow a telehealth practice" is demand-rich and supply-poor, and menopause telehealth remains untouched.
- The "found online" cluster (GBP, reviews, local SEO, practice website) is the widest open packaging gap. Demand proven at the generic level, doctor-specific supply essentially one hobbyist video deep. The first credible, well-packaged "get your practice found on Google" series owns the shelf.
- Evergreen decay is absent everywhere: 2018-2022 videos still rank across both rounds. Videos here compound for years.
- Every qualifying muse in both rounds has weak packaging (vendor demos, government slideshows, stock thumbnails, 109-sub screen-shares). Not one is a strong creator. The out-package thesis survives 27 keyword searches.
