1. Why KSM-66 for Cortisol

Ashwagandha KSM-66 has the strongest supplement evidence of any phytonutrient for cortisol reduction in human RCTs. This isn't a wellness trend, it's the most consistent finding across the adaptogens literature.

The APOE4 Reason This Matters More Than It Does for Others

For e4/e4 carriers, cortisol reduction isn't a wellness preference. It's an Alzheimer's prevention priority. The HSD11B1 mechanism means APOE4 amplifies cortisol inside neurons (specifically in the entorhinal cortex, the first region lost to Alzheimer's) even when serum cortisol looks normal.     Interpret and decide If TSH stays above 0.1 and you have no hyperthyroid symptoms: continue at 600mg/day. If TSH drops below 0.1, or if you develop hyperthyroid symptoms before then: reduce to 300mg/day or discontinue and consult your prescriber.

Both use the same KSM-66 extract. The brand matters less than the extract form. Look for "KSM-66" on the label, not just "ashwagandha." Generic ashwagandha without the KSM-66 certification uses different standardization methods and wasn't used in the RCTs that produced the cortisol data.

5. What to Watch For

Positive Signs (You're on Track)

  • Better sleep quality and faster sleep onset (often appears first, at 4 weeks)
  • Reduced anxiety or perceived stress (especially in the evenings)
  • Lower waking cortisol if you retest salivary cortisol at 8 weeks
  • More stable energy, less of the "wired but tired" pattern

Warning Signs (Reduce Dose or Stop)

  • ⚠️ Heart palpitations or racing heart
  • ⚠️ Worsened insomnia or difficulty falling asleep
  • ⚠️ Increased anxiety rather than decreased anxiety
  • ⚠️ Heat intolerance, excessive sweating
  • ⚠️ TSH below 0.1 on the 8-week retest
If any warning signs appear before the 8-week checkpoint: reduce to 300mg/day immediately. If symptoms persist, discontinue and check TSH early. Don't wait for the scheduled 8-week test if you're having symptoms.

Timing of Effects

Expect sleep improvements first, typically at 4 weeks. Cortisol effects and stress resilience build through weeks 4 to 8. Thyroid effects (if any) emerge by 8 weeks. Don't assess whether it's working before the 8-week mark.
